When I open my own PR Company, is it okay to only have an assistant and a secratary as my full staff?

Sure, it’s OK. If you really start with 20 big name clients though, you might be too busy to survive without a bigger staff. But lotsa luck in managing to start a new business with that many big name clients - or that many at all, actually, for a new PR firm.
